Amending your U.S. Passport

As an additional security enhancement, valid passports can no longer be amended. Instead, those requesting name changes will need to apply for replacement passports. If your full validity passport is less than one year old, a new passport with your new legal name can be issued without additional fees.  After one year, the fees are the same as a regular renewal.

Adding Visa Pages to Your U.S. Passport

Blank visa pages can be added to most U.S. passports free of charge. Additional visa pages cannot be added to emergency passports issued after April 8, 2002 at U.S. Embassies and Consulates abroad. Please ensure that your full-validity passport (10 years for adults and 5 years for children) is in good condition before having pages added to it.

Instructions

Complete an Application for Additional Visa Pages (DS-4085) using black ink and submit it with your passport to the ACS unit. U.S. passport holders 14 and older must personally sign the DS-4085, but need not come to the Embassy personally to request new pages.

For children under 14, one parent must sign the DS-4085 and indicate his/her relationship next to the signature in the presence of a consular officer. The parent's passport and a copy of the child's birth certificate may be requested; in some cases, additional documentation may be requested.  The child does not have to appear at the Embassy.

Note: The Passport Amendment/Validation Form (DS-19), previously used to amend passports is no longer valid.

Security at the Consular Section
All official facilities are mandated to use access control procedures for all visitors and their handbags and packages. No luggage, electronic or photographic equipment is permitted in our waiting room or on other Embassy premises.
